New Patient Forms

To streamline your visit and reduce wait times, we ask that all new patients complete their forms through our secure patient portal before their appointment. 

You will only be able to access the portal if your demographic information has already been entered into our system.
If you are unsure whether this has been completed, please contact our office at 303-834-5677.

How To Set Up Your Patient Portal Account

  1. Click this link to access the portal.
  2. On the login page, click "Register" to create an account
  3. Enter your first name, last name, and the email address we have on file. This allows the system to match your account with your record. If your details match our records, an email confirmation will be sent.
  4. Check your email. 
  5. Follow the prompts to set up your login credentials.
  6. Once your account is set up, log in.
  7. In the top banner, click the "Forms" button to access and complete all required documents.

In-Office Option

If you are unable to complete the forms in advance, please arrive 30-40 minutes prior to your scheduled appointment to complete them in our office.
